Brian Locker has been teaching systematic root cause analysis and carrying out incident investigation for the past 37 years. Since 1996, he has used the TapRooT® system of root cause analysis to help clients all around the world to achieve improved results. Most of his work has involved the nuclear, oil and gas, utility, construction, and mining industries. His biggest investigation was a 2 Billion U.S. $ loss when the Buncefield fuel storage depot outside London, England experienced a vapour cloud explosion in December, 2005. Coming to Saudi Aramco for the first time in October, 2009, Brian has worked for Upstream Continuing Excellence as a Training and Safety Consultant for the past seven years, with his main duties being root cause analysis training and incident investigation. During that period, he has taught the TapRooT® root cause analysis system to over 1,300 individuals, certifying over 500 root cause analysts within Upstream and personally facilitating numerous serious incident investigations. Brian has a B.Sc. in Physics, an Honours B.Sc. in Geophysics in 1972 and a Masters in Environmental Engineering in 1973 from the University of Western Ontario, Canada.
