I am a mentor, connector, nurse journalist, blogger who provides information to consumers, to caregivers, and members of the healthcare team so they are better equipped to navigate and work in today’s complex healthcare system. As an experienced health professional recently turned patient, my mission is twofold. First I work to equip consumers and caregivers with information, tools, and resources they need to be informed members of the healthcare team. Secondly, by listening, mentoring, educating and advocating, arm healthcare professionals with the information they can use to understand the needs of today's healthcare consumers. My passion for helping people makes me a natural mentor, teacher, and valued professional who takes the time to listen and share best practices and strategies needed to for those in need to successfully navigate and transition through the diverse and complex healthcare system. Offerings: professional mentoring services in the areas of nursing, case management, and patient/health advocacy. Production and development for national healthcare conferences, educational webinars, and workshops. Ability to secure continuing education credits on topics such as case management, patient advocacy, patient engagement strategies, patient empowerment, successful navigation and transitions of care across the care continuum. Advice and assistance on getting published, blogging and other tools that assist in the delivery of information to a wide audience.

Elaine A. Bruner, MSN, RN-BC, has over 15 years in case management practice with roles in acute and ambulatory care. She is the 2008 Award of Service Excellence recipient from the Case Management Society of America. Elaine is the co-author of ANCC Nursing Case Management Review and Resource Management. She has been an instructor for this program for a number of years. She is a published author and frequently lectures on case management, care coordination, transitions of care nationally/span>
